Gastric bypass (Roux-en-Y) surgery is the second most popular weight loss surgery in the United States, known for its effectiveness but also its invasiveness and longer recovery time. Despite its high success rate in achieving weight loss, many patients experience weight regain 8-10 years post-surgery.

Studies indicate that up to 15-35% of patients regain a significant amount of weight or fail to achieve their desired weight loss goals after surgery.

For those facing weight regain or other complications after gastric bypass, a revision procedure may be a viable option. Gastric bypass revision is a secondary surgery that modifies or corrects the original bypass, addressing issues like pouch or stoma dilation, unresolved comorbidities (such as diabetes), or complications like ulcers and strictures.

Two Methods for Gastric Bypass Revision

There are two categories of revision procedures for Roux-en-Y gastric bypass – surgical and endoscopic revision procedures.

Gastric Bypass Revision Surgery

Revision surgery typically involves one or both of the following: increasing the intestinal bypass (skipping more of the small intestine) or reducing the size of the stomach pouch and outlet. This surgery is usually performed laparoscopically, requiring a few small external incisions.

Patients typically stay in the hospital for 1-2 days, with a full recovery taking about 4-6 weeks. This method effectively addresses the structural issues contributing to weight regain and helps patients get back on track with their weight loss goals.

Endoscopic Gastric Bypass Revision (TORe)

TORe, or transoral outlet reduction, is a nonsurgical procedure that offers a less invasive alternative to traditional revision surgery. Using an endoscope (a thin, flexible tube), the procedure is performed through the mouth to place sutures that tighten the stomach pouch or the outlet connecting the pouch to the intestine. TORe can be completed in about 45 minutes and typically requires no hospital stay. Recovery is much quicker, often taking just a few days.

How Much Does Gastric Bypass Revision Surgery Cost?

The cost of surgical gastric bypass revision in the United States ranges from $10,000 to $30,000, depending on the complexity of the surgery and the location where it’s performed. Factors such as the surgeon’s expertise, the facility, and any additional treatments can also influence the price.

How Much Does TORe Procedure Cost?

TORe procedures vary, and understanding the specific type of TORe you’ll receive is crucial since outcomes can range from 3% to 15% expected weight loss. For instance, TORe with Endoscopic Submucosal Dissection (ESD) is more complex, performed by a select few bariatric endoscopists, and yields the highest weight loss rates. In the U.S., the cost of TORe ranges from $9,000 to $18,000.
